__NOTOC__

Genital or Sex Reassignment surgery is the surgical modification of existing genitals to better match the appropriate sex of a person.

MtF Surgeries
Vaginoplasty
A Vaginoplasty is a surgical procedure that is intended to construct a vaginal canal and its mucous membrane. Vaginoplasty is a general term that describes any similar vaginal surgery, often also including a labiaplasty, with the term neovaginoplasty describing the procedure of either partial or total construction of the vagina. The most common forms for MtF are described here.

Penile Inversion
In this procedure, the skin of the penis and part of the scrotal skin is used to create the internal vagina, other scrotal skin is used to create the labia and part of the glans is used to create a clitoris. The internal vaginal skin is formed around a stent and inserted into the created vaginal vault. During this procedure, the urethra is also realigned, with the target site being as natural as possible. Depending on the amount of donor tissue available, this procedure may also require skin grafts be taken from other sites in order to provide enough tissue for a sufficient vaginal canal.

Colovaginoplasty
This is much the same as a Penile Inversion, but uses a section of the sigmoid colon to provide additional tissue aiding in the creation if the vaginal canal.

FtM Surgeries
Phalloplasty
A phalloplasty is a surgical procedure intended to create a penis. There are several different methods, but the key steps are mostly the same and are what will be described here. Firstly, a graft is removed from elsewhere in the body - often the forearm, leg or the side of the chest. This graft is then 'rolled' and sutured to create the neophallus. Additional suturing and shaping is also done to create the glans. The phallus is then attached, it is also connected to a blood supply and nerve tissue is also often connected at this point. As the neophallus contains no erectile tissue an implantable erectile prostheses is often used to enable an erection.

Metoidioplasty
This procedure is intended to release and reposition the clitoral tissue to a more forward position, so it more closely resembles the location of the penis. Due to the effects of testosterone, the clitoral tissue will grow and because of this, results of up to 10 cm have been reported. Most also report a result that is erectile and sensate. The urethra is also often extended during this procedure, allowing the neophallus to be used for urinating. A scrotoplasty is often also done at this time. Vaginectomy, hysterectomy and/or oophorectomy can also be done at this time, if any of it has not previously been done.

Scrotoplasty
A scorotplasty is the surgical creation of a scrotum. This is often done by releasing the skin of the labia and using them to form a scrotal pocket. Prosthetic testes can then be introduced.
KellyShttps://tgchatroom.com/wiki/index.php/Template:MainPageMedicalSectionTemplate:MainPageMedicalSection2011-07-24T07:15:26Z
KellyS:

Intersexuality is a collection of conditions where biological sex often cannot be determined conclusively at birth because features that are normally associated with female or male are present in an unusual combination. These features can include a variety of morphological, hormonal, genetic variations which fall outside the norms.

Intersexuality differs from but is related to androgyny, although some Intersex people do identify as androgynous.

An orchiectomy is surgical procedure to remove one or both (bilateral) of the testicles and the full spermatic cords. This procedure is often done prior to GRS as it eliminates most of the bodies testosterone production. There are some variances in how the procedure is performed, depending on the reason it is being done.

Surgery
For MTF purposes or where cancer is not an issue, an incision is made in the side of the scrotum, the testicle and part of the spermatic cord is extracted. The cord is then clamped in 2 places and cut. The end of the cord is then sutured and allowed to retract. The incision is then sutured closed. The scrotal skin will often retract against the body somewhat. At no point during the procedure is the penis involved. For other purposes, the incision is made above the pubic bone, the testicle pulled through the inguinal canal, the spermatic cord clamped and sutured and then the incision is closed.
KellyShttps://tgchatroom.com/wiki/index.php/HRTHRT2011-06-26T10:54:05Z
KellyS:

__NOTOC__

Hormone Replacement Therapy, also known as as Hormone Therapy, is intended to emulate the natural levels of sex hormones of either gender. For MtF's, this therapy is usually a combination of an estrogen and an anti-androgen. For FtM's the therapy usually includes testosterone.

MtF Therapy
As mentioned in the introduction, Hormone Therapy for MtF's includes both an estrogen and and anti-androgen. The estrogen is to emulate the natural levels of estrogen found in a woman. The anti-androgen is to combat the testosterone that is present. This is discontinued after an Orchiectomy or GRS, due to the lack of naturally produced testosterone.

It doesn't undo the existing masculinising effects of testosterone, but does cause the development of female secondary sex characteristics, such as breast growth and typical female fat distribution. It also carries with it a number of side effects and risks.

Common Drugs Used
* Estrogens:
** micronized estradiol
** estradiol acetate
** estradiol valerate
** estradiol cypionate
** estradiol enanthate
** conjugated estrogens
** esterified estrogens
** ethinyl estradiol

*Anti-Androgens:
** Spironolactone
** Cyproterone acetate (More commonly used outside of the US.)

Effects of HRT
* Skin
** Skin becomes softer.
** Sebaceous gland activity lessens which lowers the amount of sebum production on the skin, leading to a less prone to the formation of acne due to the less quantity of oil that is produced.
** The skin's pores become smaller due to the low quantities of sebum produced
** Body odor (skin, sweat, and urine) will become less "metallic," "sharp," or "acrid" and more "sweet" and "musky."
** Many apocrine glands (type of sweat glands) become inactive and body odor decreases. Sebum also contributes to body odor, the production of which is reduced by anti-androgens (as described above).

* Breast development
** The nipples will become more sensitive to stimulation.
** Breast, nipple, and areolar generally takes around 4–6 years to complete, depending upon genetics, and can take as long as 10 years.
** The size of the rib cage and shoulder width also play a role in the apparent "size" of the breasts

* Hair
** Current facial hair is only slightly affected (some reduction in density, coverage, and slower growth) by anti-androgens.
** Those taking anti-androgens will have better results with electrolysis/laser hair removal than those who are not.
** Body hair will, turn from terminal hairs to vellus hairs
** New head hairs may slightly change in texture, curl, and colour.
** Eyebrow hair becomes less "bushy" or scattered.

* Fat distribution
** The body will now tend to accumulate new fat in a typically female pattern. This includes the hips, thighs, buttocks, pubis, upper arms, and breasts. The body will now tend to use the existing fat in the waist, making the waist appear.
** Subcutaneous fat increases in the cheeks and lips making the face appear puffier, appearing to round out the face.

Side Effects and Risks
There are many possible side effects related to HRT. Some are related to the estrogens, some to the anti-androgens. This is be no means an exhaustive list, but it does cover all the basics.

* Cardiovascular
** Increased blood clotting due estrogens. This leads an increased risk for thromboembolic diseases such as deep venous thrombosis and pulmonary embolism.
** It is accepted that smoking and age are likely to increase the chances of DVT.
** Oral estrogens are also more likely to cause DVT, rather than implantable, injectable or transdermal estrogens.

* Urogynecological
** The Bladder atrophies
** The Prostate atrophies
** The perineal raphe, line that runs down the underside of the penis and down the middle of the scrotum, will darken.
** Minor water retention is likely
** There is sometimes a reduction in libido, depending upon the dosage of anti-androgens.
** Spontaneous and morning erections decrease in frequency significantly, however some who have had an orchiectomy still experience morning erections.
** The testes atrophy by as much as 50%, depending on anti-androgen dosages.

FtM Therapy
For hormone therapy in FtM's, the primary, and often, only thing prescribed is testosterone. This can be delivered as in injection, transdermal patch, orally or sub-lingually. As with MtF hormone therapies, it is not without risk.

Effects of HRT
* an enlargement of the clitoris
* redistribution of body fat,
* growth of facial and body hair
* deepening of the voice
* male pattern baldness, in some
* possible shrinking and/or softening of breasts, mostly due to changes in fat tissue
* increased libido
* increased red blood cell count
* cessation of ovulation and menstruation,
* further muscle development, especially upper body
* increased sweat and changes in body odour
* coarser skin and acne (especially in the first few years of therapy)
* alterations in cholesterol and triglyceride levels

Side Effects and Risks
* Cardiovascular
** Testosterone can cause weight gain and decreased insulin sensitivity
** Testosterone redistributes the fat toward abdominal obesity, leading to increased cardiovascular risk
** Testosterone also adversely affects the blood lipid profile by causing decreases in good cholesterol and increases bad cholesterol and triglycerides
** Supra-physiological levels of androgens (generally due to abuse) are associated with significantly increased risks of strokes and heart attacks (even in the young.) More is not better!
** The most important modifiable risk factor for many men is smoking.

* Hair
** The action of testosterone on hair follicles is mainly due to the more potent androgen, dihydrotestosterone, DHT.
** With androgen therapy, genetics primarily determines how much hair will develop (and where) as well as whether male pattern baldness will develop.
** Testosterone is converted (within the cells of the hair follicle's dermal papilla) by 5-alpha reductase to DHT.

* Gynecological effects
** Menses should cease within 5 months of testosterone therapy (often sooner.) If bleeding continues past 5 months, trans men must see a gynecologist.
** Clitoromegaly occurs, and frequently reaches its apex within 2-3 years of therapy. Sizes generally range from 3-8 cm with 4-5 cm being about average. This is genetically determined, but some physicians advocate topical clitoral testosterone as an adjunct to growth before metoidioplasty.
** It is unknown whether the risk of ovarian cancer is increased, decreased or unchanged in transgender men compared to women.
** Some transgender men report a decrease in breast size with androgen therapy. This is likely it is due to loss of fat in the breasts.
** Most transgender men report a significantly increased libido.
KellyShttps://tgchatroom.com/wiki/index.php/HRTHRT2011-06-26T10:23:54Z
KellyS:

** Current facial hair is only slightly affected (some reduction in density, coverage, and slower growth) by anti-androgens.
** Those taking anti-androgens will have better results with electrolysis/laser hair removal than those who are not.
** Body hair will, turn from terminal hairs to vellus hairs
** New head hairs may slightly change in texture, curl, and colour.
** Eyebrow hair becomes less "bushy" or scattered.

* Fat distribution

* Cardiovascular

* Urogynecological

It doesn't undo the existing masculinising effects of tesstosterone, but does cause the development of female secondary sex characteristics, such as breast growth and typical female fat distribution. It also carries with it a number of side effects and risks.

Common Drugs Used
* Estrogens:
** micronized estradiol
** estradiol acetate
** estradiol valerate
** estradiol cypionate
** estradiol enanthate
** conjugated estrogens
** esterified estrogens
** ethinyl estradiol

*Anti-Androgens:
** Spironolactone
** Cyproterone acetate (More commonly used outside of the US.)

Effects of HRT
* Skin
* Hair
* Adipose tissue distribution
* Mammary gland development
__NOTOC__

Androgen Insensitivity Syndrome (AIS) is a collection of sex development disorders caused by mutations of the genes encoding the androgen receptor. The resulting disorders vary according to the structure and sensitivity of the abnormal receptor. Common forms of AIS involve a variable degree of undervirilization and/or infertility in XY persons of any gender. A person with complete androgen insensitivity syndrome (CAIS) has a female external appearance despite a 46XY karyotype and undescended testes.

Since the early 90's, scientific advances have expanded the medical understanding and management of the molecular mechanisms of the clinical features of AIS. The value of accurate and scientifically detailed information for patients is now emphasized, with physicians no longer automatically recommending surgical corrections, with an elective option now viewed as a possible but no longer necessary intervention for ambiguous conditions.
KellyShttps://tgchatroom.com/wiki/index.php/Cosmetic_SurgeryCosmetic Surgery2011-06-25T07:53:31Z
KellyS:

# Being transgender is a choice.
# Gender and sexuality are the same thing.
# All trans people are men in dresses.
# Trans people can't have families.
# All trans men are butch/extremely masculine and all trans women are extremely feminine.
# All trans women have breast implants.
# Trans women are all sex workers.
# Trans women transition just to have access to more sex partners.
# All trans people want surgery.
# You aren't a real trans person if you don't want surgery.
# Being a trans person is extremely rare (see Prevalence).
# If you don't feel like your gender was wrong from a very early age, you aren't a real trans person.
# It's always just a phase. It can be cured by religion, drugs, peer pressure, etc.
# Everyone is trans in the same way.
# Trans men transition to get male privilege and trans women try to gain female privilege while retaining male privilege.
# It is okay to allow transphobia and cissexism to continue because being trans is somehow wrong/evil/sinful.
# Psychology invented transexualism as a way to make queer and gender non-conforming people into straight, conforming people.
# Being in an unaltered body is more moral than being in an altered one.
# Being trans is just like being schizophrenic - they are both just hallucinations and delusions, so transition should be prohibited.
# Gender is either masculine or feminine; trans people are just confused between the two.
# There are no religions which accept being trans.
# Teens and younger people are not old enough to know their gender identity.
# Trans people are deceiving others about their "true" gender.
Laser Hair Removal is a process that uses a medical-grade laser to heat the hair follicle in order to destroy it. The laser selectively targets the dark matter in the follicle, so it doesn't harm the surrounding tissue. Due to the requirement for dark matter in the follicle, this process is only really effective on people with light to medium skin tones with dark hair, it doesn't work on grey hair, for example.
KellyShttps://tgchatroom.com/wiki/index.php/Voice_SurgeryVoice Surgery2011-06-21T11:14:07Z
KellyS:

This section is coming. The editor needs to do more research.

KellyShttps://tgchatroom.com/wiki/index.php/GRSGRS2011-06-19T09:37:43Z
KellyS:

Surgery
An incision is made at the inframammary fold and a pocket is made either between the chest muscle and breast tissue, or the ribcage and chest muscle. The implant is then inserted in to the pocket. The implant is filled and checked for size, position and symmetry. Once both are even, the incisions are then closed. Due to the implant being empty during implantation, a smaller incision is required than for silicon implants.

Silicon Implants
Silicon implants consist of an elastomer-coated shell filled with a semi-solid gel. Earlier generations of silicon implants had some issues with leakage, newer shell technology as well as the use of thicker fillers have reduced leakage.
A reductive mammoplasty is a surgical procedure, done under general anaesthesia, that is intended to reduce the volume of the breast. It is also used to remove the breast completely, this is called a mastectomy.

Surgery
Generally, while under general anaesthesia, an incision is made around the nipple, running down vertically to the inframammary fold. Depending on the size of the breast, the incision may also run along the inframammary fold. Excess fat, skin, glandular tissue and any breast implants are removed, the nipple is repositioned higher and the incision is closed. The initial recovery period is generally only a few weeks, but full recovery can take up to a year.
ActiveDiscuss
__NOTOC__
Facial Feminization Surgery (FFS) refers to a family of surgical procedures intended to alter the basic structures of a male face in order to bring it's appearance more in line with those of the average female. This family consists of techniques derived from both general plastic and reconstructive surgeries as well as maxillofacial procedures. The surgeries now known as FFS were originally pioneered in their current form by Douglas Ousterhout in the U.S.A during the 1990's. Facial hair removal is usally seen as a separate procedure, not included as past of FFS.

It does have it's limitations though, as surgical correction can only achieve so much. It also carries with it all the usual surgical risk as well as being quite expensive.

Surgical Procedures
Here's a list of the more common procedures used as part of FFS. It is by no means exhaustive, but covers all the major procedures. Not all of them are required to have FFS, but a combo of some of them is usual.

Brow lift
Female eyebrows are usually positioned higher on the face than on the male face. In this procedure the brows are lifted to a more classically female location.

Rhinoplasty
Rhinoplasty is the reshaping of the nose cartilage intended to alter the overall appearance of the nose. This can include reducing the length, width and thickness of the nose.

Cheek implants
Cheek implants are often used to give a more feminine appearance to the cheeks. Female cheekbones are generally fuller and have a more forward projection and this surgery aims to emulate that.

Lip Lift & Fills
Lip Lifts and Fills are often done in combination in order to improve the feminine appearance of the lips. In a lip lift, a small incision is made under the nose, a small section of skin removed and the incision is closed, reducing the distance between the nose and the lip, giving a more typically female appearance. Fillers can then be used to give fuller lips also.

Forehead recontouring
This procedure is intended to reshape the Forehead in order to give it a more typically female shape. Generally, in this procedure, a combination of grinding and filling is used to remove the brow ridge that is found on the male forehead, just above the eyes.

Chin recontouring
This procedure is intended to reshape the chin in order to give it a more typically female shape. The chin is reshaped by either bone shaving, removing larger sections of bone or both. This has the effect of reducing the height of the chin, as well as rounding out the corners

Jaw recontouring
This procedure is intended to reshape the jaw in order to give it a more typically female shape. The jaw bone is reshaped by either bone shaving, removing larger sections of bone or both. This has the effect of narrowing the appearance of the jaw, as well as rounding out the corners
KellyShttps://tgchatroom.com/wiki/index.php/FFSFFS2011-06-16T07:18:55Z
KellyS:

Rhinoplasty is the reshaping of the nose cartilage intended to alter the overall appearance of the nose. This can include reducing the length, width and thickness of the nose.

Cheek implants
Cheek implants are often used to give a more feminine appearance to the cheeks. Female cheekbones are generally fuller and have a more projection and this surgery aims to emulate that.

KellyShttps://tgchatroom.com/wiki/index.php/FFSFFS2011-06-16T07:06:49Z
KellyS:

A Tracheal Shave, also known as a Chondrolaryngoplasty, is a fairly straight-forward surgical procedure in which the thyroid cartilage is made smoother/smaller by shaving the cartilage through a small incision in the throat.

Surgery
Firstly, a anaesthetic is administered. Then a small incision is made below the trachea, the muscles and skin are retracted and a scalpel is used to shave the trachea down in order to reduce it's prominence. After the required amount of reduction is acheived, the incision is then closed. This procedure usually causes minimal scarring as the incision site is usually in one of the small folds in the neck.

This procedure can be performed under either local or general anaesthesia, depending on whether it is being performed on it's own or as part of other FFS or GRS surgeries. The trachea is located close to the vocal cords, and as such there is some risk of damage to the vocal chords. During the healing phase, difficulty swallowing and speaking is not uncommon. Recovery time is generally around 2 weeks.

Voice
This procedure will have only a minimal effect on voice, if any. During the healing phase, there may be some quite obvious changes to the voice, but these are caused by swelling from the procedure. These changes will disappear as the swelling reduces. Many people choose to have this procedure done whilst having voice surgery.
KellyShttps://tgchatroom.com/wiki/index.php/OrchiectomyOrchiectomy2011-06-14T12:33:53Z
KellyS:

For other purposes, the incision is made above the pubic bone, the testicle pulled through the inguinal canal, the spermatic cord clamped and sutured and then the incision is closed.
KellyShttps://tgchatroom.com/wiki/index.php/Breast_Augmentation_And_ReductionBreast Augmentation And Reduction2011-06-14T11:28:35Z
KellyS:

In this section, we will cover various breast-related surgeries. Both breast augmentation and breast reduction will be discussed, as well as the various surgical methods.

Breast Augmentation
Saline Implants
Saline implants consist of a silicone elastomer shell, that is later filled with saline solution. They have the benefit of being able to be implanted with a smaller incision, resulting in less scarring.

Earlier generations of silicon implants had some issues with leakage, newer shell technology as well as the use of thicker fillers have reduced leakage

Breast Reduction

Surgical results are generally quite good.
KellyShttps://tgchatroom.com/wiki/index.php/Tracheal_ShaveTracheal Shave2011-06-14T09:56:08Z
KellyS:

An anaesthetic is first administered to the patient, either local or general, and then a small, horizontal incision is made on the neck below the Adam's apple. The muscles of the throat are then retracted using forceps, and the tracheal cartilage is shaved down using a scalpel, reducing the size of the Adam's Apple. The incision is then sutured closed. Minimal scarring occurs in most cases due to the fact that most surgeons make the incision in one of the minuscule folds of skin on the neck.
